I consent, agree, and opt-in to the lawsuit filed against Beach Girls Bar, styled Turner v. Pretty Women, Inc.., et al., Case No. 4:22-cv-00085 (S.D. Iowa) (the “Lawsuit”) to pursue my claims of unpaid wages under the Fair Labor Standards Act (“FLSA”).
I certify that I am or was an exotic dancer at Beach Girls at some time since March 9, 2019 to the present.
By choosing to join this lawsuit, I understand that unless I withdraw or am dismissed from the case, I will be bound by a judgment, if any, whether it is favorable or unfavorable.
I designate the law firm of Hodges & Foty, LLP as my attorneys to prosecute my wage claim. I understand that I have the option to select another attorney of my choosing, and I voluntarily select Hodges & Foty LLP as my attorney in this matter.
I consent to having Plaintiff Cierra Turner and Plaintiff’s Counsel make all decisions regarding the litigation, including the method and manner of conducting the litigation, the terms of any potential settlement of this litigation, releasing of claims, entering into an agreement with Plaintiff’s Counsel regarding attorneys’ fees and costs, and all other matters pertaining to this lawsuit.
I understand that I may be required to actively participate in this lawsuit by answering written questions under oath, providing documents relating to my claim, answering verbal questions under oath, and/or testifying at trial or a hearing with regard to my claims against Defendants.
If needed, I authorize the Plaintiff’s lawsuits to use this consent to re-file my claim in a separate lawsuit or arbitration against Defendants.
